What is HoLEP?

Holmium Laser Enucleation of the Prostate (HoLEP) is a state-of-the-art, minimally invasive procedure used to treat Benign Prostatic Hyperplasia (BPH)—a common condition in aging men where the prostate gland enlarges and obstructs the flow of urine.

Unlike traditional surgical methods such as TURP (Transurethral Resection of the Prostate), HoLEP uses a high-powered holmium laser to remove the excess prostate tissue with greater precision and less bleeding, improving urinary symptoms and overall quality of life.

BPH can lead to significant lower urinary tract symptoms (LUTS), including frequent urination, weak urine flow, incomplete bladder emptying, and in some cases, urinary retention or kidney complications. HoLEP offers an effective, long-term solution.

How HoLEP Works

HoLEP is performed using a thin, telescope-like instrument (resectoscope) that is inserted through the urethra, meaning no external incisions are needed.

Step-by-step procedure:

  1. A holmium laser fiber is passed through the instrument to enucleate (cut away) the overgrown prostate tissue from the surrounding capsule.
  2. The freed tissue is moved into the bladder.
  3. A morcellator is used to break the tissue into small pieces and remove it from the body.
  4. The removed tissue is sent for pathological analysis to check for any signs of prostate cancer.

Because the holmium laser also seals blood vessels as it cuts, HoLEP results in significantly reduced blood loss, allowing for quicker recovery and shorter hospital stays.

Key Benefits of HoLEP

Suitable for all prostate sizes, especially large glands (>100ml)
Minimal bleeding, even in patients on blood thinners (with physician guidance)
Short hospital stay — most patients are discharged within 24 hours
Quick recovery — many return to daily activities in 1–2 weeks
Low risk of reoperation — long-term success with fewer recurrences than TURP
Preserves erectile function — does not damage surrounding nerves
Pathology sample available — removed tissue can be tested for cancer
No skin incision — performed entirely via the natural urinary tract

Studies show HoLEP provides durable relief of BPH symptoms, including improved flow rates and reduced post-void residual volumes, for up to 10–15 years after the procedure.

Types of Laser Treatments for BPH: HoLEP vs. Others

Laser therapy for BPH includes both ablative and enucleative techniques:

Procedure:

PVP/HoLAAP

Method:

Ablation with laser vaporization

Tissue Removed:

No

Best For:

Mild to moderate BPH

————————————–

Procedure:

HoLEP

Method:

Enucleation with holmium laser

Tissue Removed:

Yes (complete)

Best For:

Moderate to severe BPH

HoLEP is currently considered the gold standard for large prostates due to its completeness of tissue removal, low retreatment rate, and excellent safety profile.

Is HoLEP the Right Option for You?

Holmium Laser Enucleation of the Prostate (HoLEP) is a cutting-edge, minimally invasive surgical treatment for Benign Prostatic Enlargement (BPE). It has emerged as a preferred alternative to traditional surgeries like TURP or open prostatectomy, particularly for patients with large prostates or those seeking a durable, long-term solution with fewer side effects.

You may be a strong candidate for HoLEP if:

You Have Moderate to Severe Urinary Symptoms

Do you struggle with symptoms such as:

  • A weak or interrupted urine stream
  • Difficulty starting urination
  • Frequent nighttime urination (nocturia)
  • Urgency, leakage, or a constant feeling of incomplete emptying?

If these issues are interfering with your daily activities, work, or sleep, HoLEP may offer significant and lasting relief.

Your Prostate is Significantly Enlarged

HoLEP is especially beneficial for men with prostates larger than 80–100 milliliters in volume, where other surgical options may be less effective or carry higher risks. HoLEP can remove a greater volume of tissue compared to TURP, making it ideal for treating very large glands without resorting to open surgery.

Medications Are No Longer Working (or Not Tolerated)

If you’ve tried alpha-blockers or 5-alpha reductase inhibitors without sufficient relief—or if you’re experiencing side effects such as fatigue, dizziness, or sexual dysfunction—HoLEP offers a surgical alternative that eliminates the need for ongoing medication.

You Have Recurrent Urinary Retention or Bladder Complications

HoLEP is commonly recommended for men who suffer from:

  • Recurrent episodes of urinary retention (inability to urinate without a catheter)
  • Frequent urinary tract infections (UTIs)
  • Bladder stones or significant post-void residual urine

These conditions indicate that the enlarged prostate is severely blocking urinary flow, and surgical intervention like HoLEP can resolve the obstruction and restore proper bladder function.

You Are Looking for a Safe, Long-Lasting Solution

HoLEP has been proven to provide excellent long-term outcomes:

  • Lower risk of needing repeat surgery
  • Minimal bleeding (ideal for patients on blood thinners)
  • Short hospital stay and quick return to normal activity
  • Durable symptom control for 10+ years in most patients

Additionally, because HoLEP preserves surrounding tissue and nerves, it is less likely to affect sexual function compared to older techniques.

Talk to Your Urologist

At MedicalPoint, we use advanced imaging and diagnostic tools to evaluate your prostate health. If you meet the clinical criteria and are medically fit for surgery, HoLEP could be a life-changing solution—one that offers fast relief, low complication rates, and long-term satisfaction.

Schedule a consultation with our Urology Department to find out if HoLEP is right for you.

What to Expect: Before, During, and After HoLEP

Before:

  • You will have a consultation with a urologist and undergo imaging or urine tests
  • General or spinal anesthesia is administered on the day of surgery

During:

  • The procedure takes 1–2 hours, depending on prostate size
  • A catheter is placed temporarily to allow healing

After:

  • Most patients go home within 24 hours
  • The catheter is usually removed within 12–24 hours
  • Avoid heavy lifting or strenuous activity for about 2–4 weeks

You may experience mild discomfort, urgency, or frequent urination, which typically improve in days to weeks

Common Side Effects and Risks

Most side effects are temporary and resolve during recovery:

  • Urinary tract infection (UTI)
  • Urethral stricture (narrowing of the urethra) — rare
  • Retrograde ejaculation (dry orgasm) — common, but harmless
  • Temporary urinary leakage — usually resolves within weeks

Importantly, HoLEP does not cause erectile dysfunction. As Dr. Amy Krambeck, a world-leading HoLEP expert, states:
“If patients have good erections prior to surgery, they’ll have good erections afterward. That’s a huge advantage.”

What Makes HoLEP Unique?

Dr. Krambeck describes HoLEP as:

“Peeling an orange from the inside out — removing the pulp without touching the skin. We use a laser to remove the tissue and a morcellator to break it down like a food processor.”

This precision allows complete removal of the blocking tissue while preserving surrounding structures — a safe, efficient, and modern solution to BPH.
